Who is eligible to receive the COVID-19 vaccine?
Anyone 12 years of age and above is eligible to receive their first dose of the COVID-19 vaccine. Minors (persons between the age of 12-17) must be accompanied by a parent or legal guardian to receive the COVID-19 vaccine.

What do I bring to my appointment?
If you are receiving your FIRST DOSE of the COVID-19 vaccine, please bring your photo ID and appointment ticket (printed or digital) to your appointment.
If you are receiving your SECOND DOSE of the COVID-19 vaccine, please bring your photo ID, appointment ticket (printed or digital) and COVID-19 vaccination card to your appointment.
For 12-17 year-olds, government-issued identification is required to validate age (passport, birth certificate, etc.)
Can I walk-in without making an appointment?
Walk-ins will be accepted but will be limited to 15 per hour. In order to avoid unnecessary waiting, it is best for you to schedule an appointment.
For my first dose, can I choose which vaccine brand I receive?
Yes. For anyone 18 or over, the Pfizer-BioNTech COVID-19 vaccine or the Johnson & Johnson COVID-19 vaccine are available as options. For adolescents age 12 – 17, only the the Pfizer-BioNTech COVID-19 vaccine is authorized by the FDA.
Are there any special considerations in order for me to be eligible to receive the COVID-19 vaccine?
In order to receive the COVID-19 vaccine, you must meet the following pre-screening eligibility criteria:
Must not currently be infected with COVID-19, or previously exposed within the last 14 days.
How can I cancel my appointment?
Login to your Eventbrite account; click on your profile email on the upper right of the screen; select “tickets”. You will see the option to cancel on the left side of the screen (orange button).
